This is a screening questionnaire, not a diagnosis. A high score does not mean you have OCD, and a low score does not rule it out — only a qualified professional can diagnose. If intrusive thoughts or compulsions affect your life, please reach out to a mental-health professional.
The OCI-R adds up your 18 answers (each 0–4) into a total from 0 to 72. Higher totals mean more frequent or distressing obsessive-compulsive symptoms.
The cut score
A total of 21 or more is the usual point at which OCD is considered possible and worth a proper assessment. Below that, OCD is less likely on this screen — but a screen can miss things.
What it does and doesn't mean
This is a screen, not a diagnosis. Many people have occasional intrusive thoughts or habits without having OCD; what matters clinically is how much they distress you and disrupt your life — which a clinician assesses, not a questionnaire.
What to do next
- If your score is at or above 21, or compulsions/intrusive thoughts are eating into your time or wellbeing, talk to a GP/doctor or a mental-health professional.
- OCD is very treatable — effective therapies (like ERP) exist.
